FAITH SPECIALIZED CARE SERVICES, INC.

HISTORY

Faith Specialized Care Services Inc. was established in December 2009 by Lydia Hodge. Lydia currently owns & operates Faith A.F.C. Inc. Michigan. Lydia Hodge started her career at Family Tyes A.F.C. Home in 1993. She advanced at Family Tyes from Home Manager to Administrator. This Work experience was the foundation to the development of Faith A.F.C. Inc.

Faith A.F.C. currently has five locations with the capacity for 26 residents.

 

Faith Specialized Care Services, Inc. (FSCS) goal is to support individuals with intellectual disabilities & provide needed services & supports for persons with co-existing mental health needs

 

Faith Specialized Care Services, Inc., will utilize local community resources including public schools, libraries, local museums, shopping centers, local parks, recreation centers, & mental health services.

SERVICES

SUPPORTED LIVING

  • Provides individualized support & services that are designed to accommodate the abilities & needs of each supported individual
  • Provides Direct Support Professionals (DSPs) to give supported services to service the recipients in the supported living home.
  • Provides quality support for individuals who have intellectual disabilities and/or mental health disabilities.

COMMUNITY BASED DAY

  • Provides opportunities for individuals to participate in individualized activities that are productive & meaningful utilizing their Individualized Support Plan (ISP). 

RESIDENTIAL HABILITATION

  • Provides supports to service recipients, promoting opportunities for service recipients to make choices in life skills development, while participating in activates within the community.

FAITH SPECIALIZED CARE SREVICES, INC. STAFF SHALL

 

  • Assist the person served in activities of daily living, supportive rehabilitative services & provide them with daily on-going activities & supervision in a safe environment.
  • Accompany the person served on outings & appointments based on their individualized support plane & health care needs.
  • Administer medication as prescribed by their physician.
  • Engage the person served in participating in 'in-house" activities such as games, movies, music, arts & crafts, hygiene, social skills, communications, basic math & money management skills.
  • Engage the person served in activities within the community such as shopping, restaurants, church, movies, & community centers.
